How the Parasite Gets In

Heartworm infection begins with a mosquito. When a mosquito bites an already-infected dog (or wild canid like a coyote or fox), it ingests microscopic heartworm larvae called microfilariae. Over the next 10–14 days, those larvae develop inside the mosquito into their infectious stage. The next time that mosquito bites a dog, it deposits the larvae into the skin through the bite wound.

From there, the larvae migrate through the dog's tissue over several months, molting through developmental stages before eventually entering the bloodstream. By roughly six months post-infection, they reach the heart and pulmonary arteries as sexually mature adults — and can begin reproducing, releasing their own microfilariae into the dog's blood. What Worms Do Inside the Body

Adult heartworms settle primarily in the right side of the heart and the pulmonary arteries — the vessels that carry blood from the heart to the lungs. A single dog can harbor anywhere from a few worms to over 250, and each worm can live 5–7 years. The physical and inflammatory damage they cause accumulates over time.

Even a modest worm burden triggers an inflammatory response in the arterial lining. The pulmonary arteries thicken and become damaged, making the heart work harder to push blood through narrowed, stiff vessels. This strain leads to a condition called pulmonary hypertension — chronically elevated blood pressure in the lungs. Over time, the right side of the heart enlarges and weakens trying to compensate.

In severe cases, worms can accumulate so densely that they block major blood flow in a life-threatening emergency known as caval syndrome, which requires emergency surgical removal of worms just to stabilize the dog.

Lung tissue itself suffers as well. The body's immune system attacks the worms but also inadvertently damages surrounding tissue, leaving scarring that can permanently reduce lung function even after successful treatment.

Recognizing the Stages of Disease

Veterinarians classify heartworm disease in four stages based on severity of symptoms and diagnostic findings:

  • Class 1: No symptoms or only mild, occasional cough. Worm burden is typically low.
  • Class 2: Mild to moderate exercise intolerance and persistent cough. Some abnormal lung sounds may be present.
  • Class 3: Fatigue even with minimal activity, labored breathing, coughing, and possible fainting. X-rays typically show heart and vessel changes.
  • Class 4 (Caval Syndrome): A life-threatening blockage requiring emergency intervention before any other treatment can begin.

Because early stages are often symptom-free, many dogs are diagnosed only through routine blood testing. Don't Skip Annual Testing

Even dogs on year-round preventive medication should be tested annually for heartworm. No medication is 100% guaranteed, and a missed dose or late refill can create a window of vulnerability. A simple blood test during a routine vet visit is all it takes to catch infection early — when it's most treatable.

Why Treatment Is So Demanding

Heartworm treatment is effective but genuinely intensive — for the dog and the owner. The standard protocol involves injections of a prescription drug called melarsomine, an adulticide that kills adult worms. The drug is administered in a series of deep intramuscular injections, typically across several visits.

The critical danger during treatment isn't the drug itself — it's what happens as worms die. Dead worm fragments can lodge in small blood vessels and cause dangerous clots or inflammation. To minimize this risk, dogs must be kept strictly calm and exercise-restricted for weeks to months throughout and after treatment. This means no running, jumping, or off-leash time — even when the dog feels fine.

Treatment protocols also commonly include preliminary steps: antibiotics to target a bacterium called Wolbachia that lives symbiotically inside heartworms (disrupting it weakens the worms and reduces inflammation), preventive medication to eliminate any circulating larvae, and corticosteroids to manage inflammatory reactions.

Unlike conditions that resolve quickly, heartworm recovery is measured in months. Dogs are retested 6 months after treatment to confirm all worms are cleared.
